Research and Evaluation Resources Sponsored Living Well Iowa hosts (sites that receive funding) participate in our research program. This research helps make the Living Well Iowa program more effective by answering important questions, such as:
Research instructions: Detailed instructions for getting informed consent, completing and collecting research surveys, and preserving confidentiality. Confidentiality policy: Outlines the policy, suggests ways to orient participants to confidentiality rules, and discusses two exceptions to the rule. Consent letter: The letter and permission slip used to document informed consent by participants before they complete the questionnaire. Living Well Iowa evaluation questionnaire: Participants complete this questionnaire once before taking Living Well, and three times after the completing the course. Living Well with a Disability (LWD) Consumer Satisfaction Survey: Participants complete this during the final class (also available in MS Word). Continuing to Live Well with a Disability (CLWD) Consumer Satisfaction Survey: Participants complete this during the final class (also available in MS Word). |
