Sponsored host sites
Sponsored Living Well Iowa host sites receive funding to cover costs, including transportation of participants. Sponsored sites are required to participate in our research program. To learn more about being a host site, contact the Living Well Program Consultant.

• Research instructions: Detailed instructions for getting informed consent, completing and collecting research surveys, and preserving confidentiality.

• Confidentiality policy: Outlines the policy, suggests ways to orient participants to confidentiality rules, and discusses two exceptions to the rule.

• Consent letter: The letter and permission slip are used to document informed consent by participants before they complete the questionnaire; approved by the University of Iowa Internal Review Board.

• Living Well Iowa evaluation questionnaire: Participants complete this questionnaire once before taking Living Well, and three times after the completing the course (30-days post course, 6-months post course, and 1-year post course) (PDF).

• Living Well with a Disability (LWD) Consumer Satisfaction Survey Participants complete this during the final class (also available in MS Word).

• Continuing to Live Well with a Disability (CLWD) Consumer Satisfaction Survey
  Participants complete this during the final class (also available in MS Word).

Other host sites. Other groups and organizations may also provide the Living Well program, and they may use the information provided on this web site at no charge. However, they are responsible for the costs of providing Living Well Iowa classes, including advertising to recruit students, room rental, facilitator wages, transportation for participants, and purchasing or printing materials.
